Position Summary:
The Medical Secretary - Population Health has a passion for preventing disease, promoting healthy behaviors, promoting healthy aging, and reducing health disparities. The Medical Secretary supports the Population Health Nurse program to schedule Medicare Wellness Visits, coordinate nurse schedules, and ensure smooth, efficient operations.This program is integral to St. Luke’s efforts to design, develop, and implement successful care delivery methodologies needed for patient care coordination across the healthcare continuum, alternative payment models and value-based care. The Medical Secretary will work in active partnership with the Population Health nurses, office staff, and other health care providers to develop and implement a well-coordinated wellness program to address patient needs and ensure St. Luke’s successfully meets quality performance, enhances patient outcomes, and optimizes resource utilization.Responsible for promoting teamwork with all members of the healthcare team. Performs duties in a manner consistent with St. Luke’s mission and values.
Education, Experience, & Licensing Requirements:
Education: High School Diploma or equivalent
Experience: Previous healthcare experience is preferred
