Mindful Care is seeking a Medical Records Clerk to be responsible for organizing, maintaining, and securing patient health records within Mindful Care’s hybrid work environment. This role ensures that all medical records are accurate, complete, and accessible to authorized personnel, supporting the efficient flow of information for patient care, billing, and compliance purposes. This role is also responsible for the release of medical information to outside entities within the parameters of federal and state regulations.
Our Medical Records Clerks:
Records Management
- Retrieve, collect, and prioritize all requests for medical records made through the organization, patients, and affiliates.
- Record and track all record requests, releases, and authorizations within the Electronic Medical Record HPI
- Verify the legitimacy, accuracy, and authority of all such requests and then process and invoice as appropriate.
- Review, develop, and implement standards and procedures for processing medical records requests with administrative support
- Maintain an up-to-date understanding of applicable policies, processes, laws, and regulations.
Data Entry and Accuracy
- Verify the accuracy and completeness of records and resolve any discrepancies.
Patient Confidentiality
- Abide by, adhere to, and conform to all applicable organizational, local, state, and federal regulations including 42CFR Part 2.
- Ensure all patient records are securely stored and only accessible to authorized personnel.
- Ensure all physical information containing PHI is stored and/or disposed of in accordance with state, federal, and local laws.
Compliance and Quality Assurance
- Review records for compliance with internal and external policies and regulations.
- Report breaches, instances of non-compliance, patient complaints, problems, or similar instances to the supervisor in an effort to protect patient HPI.
- Assist in audits, inspections, and quality control checks to maintain record integrity.
Communication and Coordination
- Works closely with administration, billing, and clinical staff to support the requests from patients and outside entities with obtaining records to support the patient’s care.
- Assist patients, staff, and affiliates with medical records requests and questions.
- Respond to record requests from patients, providers, or legal representatives, ensuring proper authorization.
- Review incoming mail for health information requests or records, scan into EHR, and direct to the appropriate department.
Maintenance and Troubleshooting
- Maintain records-related equipment, such as printers and scanners.
- Report technical issues with EHR software to IT support for timely resolution.
Your Qualifications:
- High school diploma or equivalent; an associate’s degree in health information technology or a related field is preferred.
- Previous experience in medical records, healthcare administration, or a similar role.
- Familiarity with EHR/EMR systems and medical terminology.
Your Skills:
- Strong organizational skills, attention to detail, and the ability to handle confidential information responsibly.
- Ability to be professional and courteous with internal and external customers at all times, including under stress.
- Ability to work both independently and as a part of a team.
- Ability to communicate articulately and comprehend written and verbal communications.
Position & Schedule Type:
- Type: Full-time, non-exempt
- Schedule/Hours: Monday – Friday 9 AM – 5 PM
- Location: In-Person, West Hempstead office
