Position Description:
The Medical Records Clerk is responsible for support of the medical records function including review and processing of medical records requests, adherence to HIPAA regulations and all applicable state, federal and local laws pertaining to the production of medical records; developing and maintaining various tracking reports and application of state law requirements pertaining to production of medical records. The Medical Records Clerk is responsible for responses to subpoena’s, third party records requests and documentation of the same, as well as ensuring that all productions or disclosures are HIPAA compliant.
Hours: Monday - Friday, 10:00 am - 2:00 pm
Pay Range: $18.50-19.50/hour (pay is commensurate with experience)
Essential Functions:
· Responsible for daily review of incoming medical records requests and identification of type of requests
· Responsible for ensuring medical records requests meet legal and regulatory requirements
· Responsible for timely processing medical records requests
· Responsible for invoicing of medical records and reconciling with corresponding requests
· Responsible for maintaining new and archived medical records and/or charts
· In concert with the Medical Records Manager maintain tracking logs and reports regarding requests and status
· Coordinate the response to third party medical records requests; compile information for insurance company audits
· Facilitate and adhere to storage log and destruction procedures for outdated records in accordance with Professional’s procedure for record retention and destruction
· Determine the appropriate release of medical records information and prepare correspondence to respond to requests in accordance with Company policies and all applicable federal, state and local laws
· Participate in internal meetings regarding medical records and protection of patient information
· Ensure compliance with medical record maintenance standards; ensure best in class standards for medical records storage and destruction are met
· Ensure medical record information stored in the Company’s database is accurate and up-to-date
· Assist in regularly schedule medical records audit
· Assist in external medical records storage reports and assist in retrieval process
· Attend Company meetings as required
· Attend training classes as directed; complete assigned training courses as directed

· Maintain working knowledge of HIPAA, OSHA, Risk Management and Compliance regulations
· Practice confidentiality and privacy protocols in accordance with Company policies and all applicable laws and regulations
Requirements
Qualifications:
· A minimum of one (1) year of medical records experience required
· Working knowledge of Microsoft Office (Word, Outlook, and Excel) is required
· Working knowledge of HIPAA regulations is required
· Must be customer-service oriented and have excellent interpersonal skills
· Must have excellent organizational and time management skills; must ensure that Medical Record Information requests are responded to promptly and are properly stored or destroyed
