Medical Discharge Planning Specialist Highlights
- The pay for this position is $18.34 per hour.
- Schedule: Monday to Friday, 8:00 AM to 5:00 PM. Training will be 8:00 AM to 5:00 PM for 3 weeks (virtual).
- Remote position with the possibility of future onsite visits.
- This position is a Full-Time position with a potential for permanent placement.
Responsibilities
- Care Coordination & Discharge Planning: Collaborate with utilization management clinicians, hospital discharge planning staff, and other health program supervisors to facilitate smooth discharge plans for members.
- Medical Appropriateness & Quality of Care: Consult with the appropriate staff to ensure that quality of care is maintained and resolve any issues that may arise regarding member health outcomes.
- Negotiation & Motivational Interviewing: Engage and motivate members in healthcare decision-making and lifestyle changes to ensure the best health outcomes.
- Member Support: Guide and empower members to make informed decisions about their healthcare, including providing advice on medical care and healthy lifestyle choices.
- Collaborate on Discharge Plans: Assist with the development and execution of discharge plans, helping with home healthcare (HH) and durable medical equipment (DME) issues.
- Problem-Solving & Critical Thinking: Use strong critical thinking to identify, escalate, and resolve quality of care concerns efficiently.
- Administrative Duties: Maintain accurate records and documentation to ensure timely and complete care coordination.
Requirements
- Education: High School Diploma or GED required.
- Typing Skills: Must be proficient in keyboard typing at 45 words per minute (WPM) with a minimum score of 6,500 alphanumeric keystrokes.
- Experience: Preferably 1-2 years of experience in customer service, case management, or discharge planning, with a background in healthcare or related fields.
- Medical Terminology: Basic understanding of medical terminology.
- Critical Thinking & Communication: Must possess critical thinking skills, and the ability to communicate and collaborate effectively with members and healthcare providers.
- Customer Service: Strong customer service skills and the ability to work in a team-oriented environment.
- Technology Skills: Comfortable navigating various computer systems for case management and member outreach.
