About the Job
We are hiring a FULL-TIME( 35 hours per week) MEDICAL CASE MANAGER for our IRAP program with Catholic Charities to help our clients in the city of Buffalo at our Herkimer Office.

Our programs include comprehensive counseling services for all ages, basic emergency assistance including operating 8 food pantries, mental health services, chemical dependency treatment, educational and job readiness services, services to older adults, marriage counseling, immigration and refugee assistance and a variety of programs that address prevention and treatment of a number of family issues.

JOB SUMMARY: Responsible for coordinating service provision to individuals enrolled in program. Provides services directly or links clients with other service providers.
ESSENTIAL FUNCTIONS:
- Determines and documents eligibility for services
- Enrolls clients into program
- Develops self-sufficiency plans
- Provides direct service delivery
- Provides appropriate referrals/linkages to medical providers
- Coordinates medical appointments for clients
- Transports clients to appointments as needed
- Develops and updates service strategy plans
- Works with Case Manager to assure that client's basic needs are met
- Documents activities per contract requirements
- Prepares reports required by funding sources
- Inputs data in appropriate database(s)
- Collaborates with other community agencies
- Performs other duties as assigned
EDUCATION: BA/BS or Associates Degree with 2 years relevant experience Bi-lingual ability a plus, preferably: Swahili, Arabic, Burmese, Kirundi, Somali, or Tigrinya
EXPERIENCE: Must be able to work in a multi-ethnic, multi-cultural setting. Able to function as part of a team working with employment counselors, case managers and interpreters. Must possess a valid NYS Driver's License and vehicle. Evening and weekend availability required. Microsoft Office skills required.
