Job Description:
As a Medical Case Manager, you will play a pivotal role in the rehabilitation and recovery of injured workers within a designated jurisdiction. You will collaborate closely with patients, their families, medical providers, and our team to ensure comprehensive care and optimal outcomes.
Responsibilities:
Provide Medical Case Management through both in-person and telephonic interactions with patients, physicians, and other healthcare providers.
Utilize your medical expertise to assess and discuss treatment plans, considering alternatives for optimal patient care.
Evaluate the appropriateness, medical necessity, and cost-effectiveness of treatment plans.
Assess, plan, implement, and evaluate patient progress to support recovery goals.
Attend appointments with doctors, home visits, discharge planning conferences, and occasional attorney meetings to coordinate care effectively.
Facilitate the delivery of necessary medical equipment and nursing services.
This role involves significant local travel, approximately 60% of the work week/month, with potential for occasional overnight stays.
Requirements:
Effective communication skills and ability to thrive in a fast-paced, team-oriented environment.
RN licensure with a background in Medical Case Management, orthopedics, neurology, or rehabilitation preferred.
Experience in cost containment, utilization review, or managed care is advantageous.
Strong interpersonal, time management, and organizational abilities.
Proficiency in MS Office, including Excel, and ability to adapt to technical tools.
Capability to work independently and collaboratively within a team.
Education & Experience:
Graduate of an accredited school of nursing.
Current RN licensure in [State].
Certification as a CCM, CIRS, or similar Case Management certification preferred.
Valid driver's license, reliable transportation, and flexibility to travel as required.
