Medical Case Manager - TAG MedStaffing

Job Overview - Medical Case Manager:
Our client in the Bronx, NY, is seeking a Medical Case Manager to manage a caseload of around 22 clients. In this role, you’ll conduct comprehensive assessments, develop service plans, and provide ongoing support through home visits and fieldwork to promote client well-being and independence.

Compensation: $24/hour
Location: Bronx, NY
Schedule: 9:00 AM to 5:00 PM, Sunday to Thursday

Responsibilities as the Medical Case Manager:

• Case Management: Provide effective case management to ensure clients receive appropriate care and services.
• Assessments: Conduct assessments and develop service plans with mutually agreed-upon goals.
• Client Support: Make home visits and maintain regular contact with clients to monitor their progress and needs.
• Coordination: Assist clients in accessing medical care, medication adherence, employment training, and other support services.
• Crisis Intervention: Provide immediate support during crises, adjusting services as necessary.
• Documentation: Maintain accurate records of client interactions and ensure compliance with required documentation.

Qualifications for the Medical Case Manager:

• Education: Bachelor's Degree required.
• Experience: 1-2+ years of Fieldwork with HIV/AIDS required (OMH, mental health, or substance abuse experience preferred).
• Skills: Knowledge of case management principles, organizational skills, and strong communication abilities.
• Fieldwork: Must be comfortable with home visits and fieldwork.
• Language Skills: Bilingual (Spanish) preferred.
