Job Overview - Medical Case Manager:
Become a pivotal member of our clients team as a Medical Case Manager in the Bronx, NY. In this role, youll manage a caseload of approximately 22 clients, ensuring they receive necessary support for independent living, including assessments, service planning, and collaboration with healthcare providers.
Compensation: $25/hour
Location: Bronx, NY
Schedule: Flexible Shifts, 9:00 AM to 5:00 PM
Responsibilities as the Medical Case Manager:
Become a pivotal member of our clients team as a Medical Case Manager in the Bronx, NY. In this role, youll manage a caseload of approximately 22 clients, ensuring they receive necessary support for independent living, including assessments, service planning, and collaboration with healthcare providers.

Responsibilities as the Medical Case Manager:
- Client Management: Develop comprehensive assessments and individualized service plans, identifying both short- and long-term goals for each client.
- Fieldwork: Conduct home and office visits to monitor client progress, with a minimum of one home visit per month.
- Coordination: Assist clients in accessing primary medical care, medication adherence, daily living activities, and recreational and employment services.
- Crisis Intervention: Provide immediate support during crises and maintain awareness of clients' changing medical conditions.
- Documentation: Maintain accurate records of all client interactions, updates, and service plan revisions.
- Education: Bachelor's Degree required.
- Experience: 1-2 years of Fieldwork with HIV/AIDS populations and individuals with mental health or substance use issues required.
- Skills: Strong organizational, communication, and case management skills, with the ability to manage multiple tasks effectively.
- Technical Proficiency: Basic knowledge of Microsoft Excel and Word required.
- Preferred: Bilingual in Spanish and experience with OMH Scattered Site Programs a plus!
