Responsibilities as the Medical Case Manager (Bilingual Spanish):
- Client Advocacy: Conduct intakes and assessments, develop service plans, and connect clients to medical, mental health, and other essential services.
- Progress Monitoring: Maintain consistent contact with clients through home visits and office meetings to track progress and update service plans.
- Healthcare Coordination: Assist clients with accessing healthcare benefits, accompany them to appointments, and advocate with external organizations.
- Housing Oversight: Monitor living conditions during monthly home visits, report repair issues, and collaborate with the Housing Specialist.
- Documentation: Keep accurate, audit-ready client records, update program databases, and contribute to monthly reports.
- Language: Bilingual in Spanish required.
- Education: Bachelors degree required.
- Experience: Minimum 1 year of experience working with individuals facing homelessness, mental illness, or substance abuse.
- Skills: Excellent written and verbal communication, organizational skills, and proficiency in Microsoft Office Suite.
- Attributes: Detail-oriented, organized, and dedicated to empowering clients to achieve stability.
