• One full year of employment experience within the last three years (names and dates of places of employment must be provided)
• Current BLS - Healthcare provider certification (we can not accept online-only courses)
• Physical within the last 6 months that states you are "free from" or "without evidence of" communicable diseases
• Current PPD or Chest X-ray (we can provide PPD if needed)
• Two professional references (RN's or LPN's that you have worked with before)
• Criminal history report with no activity listed (we will obtain at no cost to you)
• *Additional credentials may be required upon hire or per contract
