MDS Coordinator at Ridgewood Manor

AOM Healthcare is a premier long term care company, operating twenty one long term care facilities throughout the state of Ohio and is now hiring a Full-Time MDS Nurse at Ridgewood Manor, located at 3231 Manley Road, Maumee, Ohio.

Job Functions & Requirements: 

• Complete accurate assessments, MDS care plan as assigned
• Initiate care plans and supporting activities as assigned
• Create and distribute monthly care plan calendars in a timely fashion
• Maintain and update all care plans and assessments as required
• Monitor audit clinical records, ensuring accuracy timeliness
• Inform DON of persistent issues related to non-compliant documentation and protecting the confindntiality of Resident Facility information at all times
• Assist the Administrative Director of Nursing with ensuring that documentation in the center meets federal, state, and certification guidelines.
• Coordinates the RAI process assuring the timeliness, and completeness of the MDS, CAAs, and Interdisciplinary Care Plan.
• Active RN license in the state of Ohio. 
• Other duties as assigned.
