Job Summary:

The MDS Coordinator (RN) is responsible for overseeing the completion of the Minimum Data Set assessments in compliance with federal and state regulations. This role involves:

Conducting resident assessments and care planning

Collaborating with interdisciplinary teams to ensure quality care

Training staff on MDS processes and documentation

Monitoring compliance and quality improvement initiatives

Maintaining accurate records and reporting

Qualifications:

Current RN license in Massachusetts

Experience in long-term care and MDS processes preferred

Strong communication and organizational skills
