Job Summary: The MDS Coordinator will oversee the assessment and care planning process for residents in a long-term care facility.

Key Responsibilities:

Conduct comprehensive assessments using the MDS tool

Collaborate with interdisciplinary teams to develop care plans

Ensure compliance with state and federal regulations

Provide training and support to nursing staff

Monitor resident outcomes and quality measures

Qualifications:

Current RN license in Iowa

Experience in long-term care and MDS processes preferred

Strong communication and organizational skills
