The MDS Coordinator (RN) is responsible for overseeing the assessment and care planning process for residents in a skilled nursing facility. This role ensures compliance with regulatory requirements and enhances the quality of care provided.Responsibilities:

Conduct comprehensive assessments of residents

Develop and implement individualized care plans

Collaborate with interdisciplinary teams to ensure optimal care

Monitor and evaluate resident progress and outcomes

Ensure compliance with state and federal regulations

Requirements:

Current RN license in Massachusetts

Experience in long-term care or skilled nursing

Strong knowledge of MDS and care planning processes
