- The role of the MDS Coordinator is to coordinate the development and completion of the resident assessment in accordance with the requirements of the state/government regulations and facility goals and policies.
- The role of the MDS Coordinator is to coordinate the development & completion of the resident assessment in accordance with the requirements
- Coordinate the development & completion of the evaluation of resident in accordance with the provisions of the regulations, facility goals, & Policies
- Participate in Continuous Quality Improvement
- Audits MDS, CAAS, PPS, QRP, & QM reports for continued compliance
- Participate in facility surveys - NYSDOH, OSHA, Infection Control, Safety, etc.
- Conduct/coordinate the interview of each resident for the resident assessment instrument & ensure they are completed
- Obtain medical records to ensure correct information for accurate completion of MDS / SNF PPS
- Ensure that cut letters are provided to residents/representatives as per regulations
- Communicate/coordinate significant change, determine skilled level of care & assure OMRA documentation as needed
- Assure assessment reference dates ARD are based on the regulatory timeframe for completion of assessment & capture intensity of care required by the resident to ensure payment at the RUG level entitled
- Clearly communicate alert to changes in payer status & compliance with transmission/submission targets
- Ensure compliance on Med A Certification Forms
Requirements:
- Must be a graduate of an approved school of nursing
- Current license in IL as RN, Registered Nurse or LPN, licensed practical nurse
- Minimum of 3 years' experience in hospital or long-term care
- 1 year experience with MDS, PRI, & CCP
- MDS certification & PRI certification, preferred
- Willingness to work harmoniously with others & do the job with detailed focus & able to continually learn current knowledge as it relates to LTC & MDS
- Excellent interpersonal & organizational skills
- Team player with clinical skills & the ability to communicate effectively with diverse individuals
