MDS Coordinator (LPN/RN). - Blue Circle Rehab and Nursing.

We are seeking an experienced MDS Coordinator, requiring a licensed practical or registered nurse. This full-time position offers a starting pay of $25.00 per hour and includes benefits such as 401(k) with matching, health and dental insurance, life insurance, paid time off, and a referral program.

Responsibilities:

• Participate in the admission process and assess nursing needs for prospective residents.
• Determine Resource Utilization Groups (RUGs) and associated expenses for admissions.
• Review admission applications and conduct assessments, communicating results to the Admissions Coordinator.
• Ensure accuracy of the MDS process for all residents.
• Maintain knowledge of Medicare criteria and communicate regulatory changes to nursing staff.
• Monitor Case Mix Index (CMI) scores for Medicaid reimbursement implications.
• Facilitate problem-solving for complex admissions.
• Ensure timely submission of Medicare assessments and nursing documentation.

Qualifications:

• Education: Associate degree (preferred).
• Experience: 2 years in MDS (required).
• License: Compact State Nurse License (required).

Work location is in-person, with an 8-hour day shift schedule.
