Job Summary:
The MDS Coordinator (LPN) is responsible for managing the Minimum Data Set (MDS) assessments and ensuring compliance with federal and state regulations.

Key Responsibilities:

Conduct MDS assessments and care planning for residents

Collaborate with interdisciplinary teams to optimize resident care

Ensure timely and accurate documentation

Provide training and support to nursing staff

Monitor quality indicators and implement improvement plans

Qualifications:

Current LPN license in Massachusetts

Experience in long-term care preferred

Strong communication and organizational skills
