Job duties:
• Transcribe and take appropriate action on physician’s orders as directed including but not limited to Care Plan changes.
• Assist with performing all admitting and discharge assessments, and complete all other necessary paperwork.
• Assist/direct visitors.
• Clean equipment.
• Help monitor all aspects of patient care.
• Initiate requests for consultation or referral. Respond to requests from the resident, physician, or nursing staff.
• Cooperate with other resident services when coordinating nursing services to ensure that the resident’s total regimen of care is maintained
• Coordinate and prepare MDS assessments for all residents.
• Oversee the transmission of MDS resident assessments.
• Prepare, monitor, and update assessments, Care Area Assessments, and Care Plans, as necessary.
• Participate in Care Plan meetings with residents and family members.
• Be aware of residents’ exit to and reentry from hospitals
Minimum Education:
MDS Coordinator will have completed all educational requirements as required for licensure and/or certification.
Minimum Work Experience:
The facility prefers that all employees have prior health care experience and be knowledgeable of the essential job functions they will be performing.
It is preferred that employees have a minimum of six months of long-term care experience.
Certification:
• It is preferred that the MDS Coordinator be certified in MDS 3.0.
