We are seeking a highly motivated and experienced MDS Coordinator to join our dynamic team in the medical industry. As a Permanent MDS Coordinator, you will be responsible for coordinating and overseeing the development and implementation of care plans for our long-term care and skilled nursing patients. You will also be responsible for ensuring compliance with state and federal regulations and providing support to the rehabilitation team. - Monday - Friday
- Shared Call
Responsibilities:
- Coordinate and oversee the development and implementation of care plans for long-term care and skilled nursing patients
- Ensure compliance with state and federal regulations regarding patient care
- Provide support and guidance to the rehabilitation team
- Conduct patient assessments and develop individualized care plans
- Monitor and evaluate patient progress and adjust care plans as needed
- Communicate with healthcare providers and family members to ensure the best possible care for each patient
- Maintain accurate and up-to-date patient records
- Participate in quality improvement initiatives and staff education programs
- Assist with the training and orientation of new staff members
Qualifications:
- Registered Nurse with a current license to practice in the state of employment
- Experience in long-term care and skilled nursing settings
- Experience with clinical reimbursement, care planning, MDS 3.0 and case mix
- Strong knowledge of state and federal regulations regarding patient care
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong organizational and time management skills
- Proficient in the use of electronic medical records and other healthcare technology
- Ability to maintain confidentiality and comply with HIPAA regulations
