Job Summary: The MDS Coordinator ensures the accurate and timely completion of Medicare/Medicaid case-mix documents to secure appropriate reimbursement for care services. This role involves conducting ongoing Minimum Data Set (MDS) reviews, overseeing MDS documentation and submission, and integrating various services to ensure proper assessment and reimbursement.

Essential Duties:

• Assess health status and care levels for new admissions.
• Complete MDS Assessments accurately and on time.
• Coordinate interdisciplinary participation in MDS completion.
• Maintain MDS assessment schedules and ensure thorough documentation.
• Conduct resident care conferences and assist in care plan formulation.
• Evaluate and revise care plans as needed.
• Manage level of care changes and notify relevant departments.

Qualifications: Registered Nurse with current state licensure; long-term care experience preferred; proficiency in English.

Physical Demands: Requires regular sitting, standing, bending, and lifting up to 25 pounds.
