MDS Coordinator - Kings Harbor Multicare Center

We are looking for full time MDS Coordinator to join our team!  

Candidates must possess the following:

§  Nursing or related degree from an accredited college or university.

§  Current RN license in the State of New York

§  Knowledge of MDS/CAAs/CCP process preferably with experience as an RN Manager or MDS/CCP Coordinator.

§  1 year experience in long term care setting.

§  PRI Certification.

The following details are some of the responsibilities for the MDS Coordinator:

§  Evaluates all new admissions, re-admission and bed retention returnees within 24 to 72 hours for Medicare/ Medicaid eligibility and/or significant change in status, and informs Director

§  Ensures the MDS 3.0 is correctly dated with assessment reference dates

§  Obtains and maintains a current and up to date list of all MDS 3.0 and CCPs due

§  Evaluates all new admissions, re-admission and bed retention returnees within 24 to 72 hours for Medicare/ Medicaid eligibility and/or significant change in status, and informs Director

§  Obtains and maintains a current and up to date list of all MDS 3.0 and CCPs due

§  Responsible for the completion of the MDS, CAAs and CCPs (excluding new admissions) and brings completed MDS from charts to Resident Assessment Department for data entry.
