MDS Assistant / Restorative Nurse - Aperion Care Elgin

Job Summary: The MDS Assistant ensures accurate and timely completion of Medicare/Medicaid case-mix documents for appropriate reimbursement. This role integrates various services to assess and monitor resident care, while overseeing the restorative nursing program and acting as a liaison between therapy and nursing.

Essential Duties:

• Assess health status of new admissions and coordinate MDS completion.
• Maintain MDS assessment schedules and ensure accurate data entry.
• Evaluate patient care and monitor nursing practices.
• Oversee restorative nursing measures and collaborate with therapy departments.
• Participate in care planning and staff education.

Qualifications: Registered Nurse with current state licensure and preferred long-term care experience.

Physical Demands: Requires regular sitting, standing, bending, and lifting up to 25 pounds.
