As a Marketing Intake Liaison at Columbia River Home Health, you will play a vital role in supporting our field liaisons and driving growth in admissions for our extensive home health and hospice services. Working closely with our field liaisons, your primary focus will be on meeting the needs of our customers and building strong relationships with referral sources to increase admissions and foster engagement. Your efforts will contribute to the smooth operations of clinical activities by processing referrals and coordinating patient care, ensuring compliance with all regulations and policies, and maintaining valuable community relationships. Key Responsibilities:
- Collaborate closely with our field liaisons to provide essential support in their efforts to educate and engage referral sources, ultimately driving growth in admissions.
- Utilize a consultative sales approach to educate medical professionals about Columbia River's home health and hospice services, ensuring they are well-informed about the solutions we offer.
- Address customer needs and provide exceptional customer service to build strong and lasting relationships with referral sources, supporting the success of our field liaisons in increasing admissions.
- Facilitate effective communication and coordination with patients, families, and physician offices to process referrals and efficiently coordinate patient care.
- Process and coordinate referrals received by the organization, ensuring a smooth and timely transition for patients into our home health and hospice services.
- Maintain and nurture valuable community relationships, representing Columbia River as a reliable and reputable provider of home health and hospice services.
- Ensure strict adherence to all relevant regulations, policies, and procedures, guaranteeing high-quality care and ethical practices.
- Analyze territory and develop call routing strategies to establish correct targets, reach, and frequency for maximal territory growth. Actively call on new referral sources to expand our customer base.
- Utilize provided resources and technology as a vehicle for success, enhancing productivity and customer engagement.
- Leverage the CRM tool on a daily basis to capture customer needs and strategize for continued hospice utilization. Synchronize updates daily for accurate and up-to-date information.
- Utilize sales tools available on provided devices as visual aids and learning tools to grow knowledge and effectively communicate with customers.
- Demonstrate proficiency in professional verbal, written, and electronic communication, ensuring clear and concise interactions with all stakeholders.
- Collaborate with internal teams, including clinical and administrative staff, to enhance communication, coordination, and achieve shared goals.
- Resolve customer feedback by investigating opportunities for development, developing appropriate solutions, preparing reports, and collaborating on resolutions with program management.
- Apply training and leverage tools and resources to execute strategies with customers, demonstrating a strong sense of urgency and dedication to delivering exceptional service.
- Be open to and proactively apply coaching feedback from direct managers, with the intention of continuously improving various skill sets.
- Ensure timely completion of administrative duties, including expense reports, mileage reports, marketing reports, and other administrative actions, adhering to required timelines.
- Work cohesively and leverage opportunities with internal customers, such as Columbia River's Director of Clinical Services, Clinical Supervisors, Clinical Field Staff, and Columbia River's Leadership, to ensure seamless operations and coordinated efforts.
