Position Overview: As a Community Case Manager, you play an essential role in supporting the well-being of children. teens and families in their community. This position offers flexible work hours, including some evening availability to best meet client needs. You'll work in close partnership with therapists, ensuring a collaborative approach to each child's success. You will have the opportunity to facilitate group sessions, providing a safe space for children and teens to develop important social and emotional skills while having fun, meaningful experiences. Primary Responsibilities: As a Community Case Manager, you will:
- Provide individualized services that support each child/teen's treatment goals.
- Act as an advocate and link to necessary community services.
- Facilitate group sessions that focus on social skills, problem-solving, conflict resolution, and other key areas of emotional and behavioral development.
- Offer education, support, and skill-building in areas like social skills, problem-solving, conflict resolution, emotion/behavior management, and coping strategies.
- Help children and teens develop skills for success in school, at home, and within their communities.
- Maintain accurate and timely records of service delivery.
Qualifications: Applicants should meet the following criteria:
- At least 21 years of age.
- Experience and knowledge of working with children and teens preferred.
- High school diploma or GED.
- Ability to pass a background check.
- Valid driver's license, safe driving record, and required auto insurance coverage.
- A well-maintained vehicle for travel within the community.
