LVN Service Care Manager - A-Line Staffing Solutions

A-Line staffing is hiring a Case Manager in Edinburg, TX! This position has career growth potential with full time / 40+ hours per week.

*MUST have an LVN License*

Case Manager Details and Compensation:

• Location: 100% hybrid - Mostly remote with home visits as needed in the Edinburg, TX area!
• Payrate: $24 - $26 per hour PLUS mileage reimbursement
• Required Availability: Monday - Friday 8am - 5pm

Case Manager Responsibilities:

• Conduct in-home face to face member assessments as needed in the area and telephonic outreach
• Follow up calls with members
• Coordination with multi-disciplinary teams
• Document assessments and contacts, develop and approve service plans, and ensure authorizations are completed.
• Assess member’s medical, safety and support service needs and coordinate the delivery of services. Provide education related to member’s health condition and care, medical consenter, or caregiver, Department of Family and Protective Services caseworker and family, if applicable, as well as the managing physician.
• Coordinate member care through collaboration with community outreach and member advocates, assigned service coordinators, and/or case managers.
• Educate providers and community resources on program components and available support services.

Case Manager Requirements:

• LVN License is required
• Verifiable High School Diploma or GED Equivalent
• Computer proficient in Microsoft Office programs and Microsoft Teams

Preferred Skills/Experience:

• 2 years of related Experience with Care Management or Services Coordination with a managed care company is highly preferred
• Bilingual in English and Spanish is highly preferred
