Job Summary:
CareLinx by Sharecare is looking for Per Diem Licensed Vocational Nurses/ Licensed Practical Nurses (LVN/LPNs) to join our team.
LVNs/LPNs have the opportunity to provide care to patients in a variety of settings including skilled nursing facilities, assisted living facilities, patients' homes, senior living centers, and hospital/clinical settings. We offer various shifts including day, night, weekdays, and weekends – choose the shifts to fit your schedule.

Qualifications:
- Three (3) months of nursing experience.
- Completion of an infusion therapy education program, ongoing training and skills competency are in the LPN/LVN employee’s file. This education program meets the state requirement if applicable. (Preferred if not State Mandated).
- Ability to communicate in English
- Active Nursing License/Certification
- Trained and experience in using PCC (PointClickCare) is preferred.
- Active CPR/BLS certification
- Proof of COVID vaccination, including the first booster
- Medical requirements including physicals and TB test may be required.
- Additional documents can be requested due to state laws and regulations, and some training requirements may be needed.

Essential Job Functions:
- Provide care within your scope of practice potentially including helping with the preparation, implementation and continuing evaluation of the patient care plan; collecting, recording, and monitoring patient data, routine measurements, and vital signs; and administering immunizations and therapeutic injections as ordered by providers
- Communicates with the care team as needed.
- Completes, maintains, and submits accurate and relevant documentation.
- Supervises ancillary personnel and delegates responsibilities when required.
