Position Overview:
Under the general direction of a qualified registered nurse , the Licensed Vocational Nurse assists in the nursing process by instructing patients and care givers in self care and providing basic nursing care as needed.
Job Description :
• Coordination care among team members and documentation in the clinical record per policy
• Educates and trains patients and caregiver on exercises and equipment needed to aid recover
• Provides education and instruction specific to patient and caregiver assessed needs, abilities and readiness
• Accurately documents ongoing assessments, clinical findings, skilled care and plan for next visit according to the established care plan
• Must be flexible with schedule including, but not limited to, the ability to participate in on call and/or department rotation for night and weekend coverage
Quality Assurance and Performance Improvement: (includes documentation and regulatory standards
utilizes appropriate body mechanics, safety and infection control measures for patient and staff protection
• Reports patient progress toward goals to the Clinical Manager or Primary Care Clinician on a regular basis
• Reports variances from medical plan of treatment and/or nursing care plan to Clinical Manager or Primary Care Clinician
• Documentation reflects coordination of activities
• Communicates effectively with team members, families, patients, physicians, referral sources and community agencies
• Demonstrates knowledge and appropriate utilization of internal, community resources to meet patient needs
• Demonstrates effective Integrated Care Management principles in clinical practice
• Demonstrates knowledge of and actively participates in the Quality Assurance and Performance Improvement program
• Documents care provided and completes documentation timely per agency policy
• Perform peer audits as assigned
• Functions as a positive role model for and assists in orientation of new personnel
• follows all applicable licensure regulations and commonly accepted professional standards of practice
Fiscal Responsibility (includes productivity, HHRG rating, supply management, utilization review)
• Demonstrates ability to prioritize individual position requirements, effectively using work time
• Accepts clinical assignments as designated by the manager
• Effectively schedules visits to meet patient need and agency productivity standard
• Submits documentation in a timely manner and in compliance with relevant agency policies and procedures
• Obtains supervisor approval for all overtime prior to the occurrence in accordance to agency policy and procedure
EDUCATION

Graduate of an accredited school of Vocational Nursing.
CERTIFICATION & LICENSURE
LVN-Licensed Vocational Nurse
BLS-Basic Life Support Healthcare Provider
CPTI-Certified Phlebotomy Technician I within 30 Days of hire
DL-Valid Drivers License Class C
AUTO-Automobile Insurance
PREFERRED EXPERIENCE AS TYPICALLY ACQUIRED IN:
1 Minimum 1 year of professional experience required.
Home health or community health experience preferred.
5 Direct patient care experience within the past five years required.
SKILLS AND KNOWLEDGE
Basic competencies in the fundamentals of nursing and pharmacology are required.
Demonstrated skills in effective written and verbal communication.
Ability to recognize the needs and concerns of diverse groups of people.
Ability to maintain harmonious constructive working relationships with internal and external customers (patients, families, members of the interdisciplinary team, clerical and management staff, physicians and community resources).
Ability to use and give instruction of adaptive equipment.
Must possess a current California drivers license and access to a reliable automobile with proof of liability insurance.
Must be able to handle sensitive issues, conflict with or among others, respectively direct and reinforce staff efforts; effectively plan and organize and prioritize work, think critically to both effectively plan and organize department operations consistent with Sutter Care at Home’s strategic and operating objectives and to effectively solve unique problems as they arise or identify when to consult supervisor.
Must be able to deal with challenging work environment with time demands and occasional conflicting priorities.
Maintain a positive and professional image and demonstrate self-directed learning to meet professional development and/or license requirements.
Responsible for maintaining all required licensure and certifications.
At orientation and annually thereafter, must be able to meet Sutter Care at Home core competencies for position and area of specialty, as appropriate.

_The salary range for this role may vary above or below the posted range as determined by location.
