You will provide routine healthcare to patients at the patient's home or in a care facility in Napa County and Solano Counties.

You will provide routine healthcare to patients at the patient's home or in a care facility in Napa County and Solano Counties.
Job Description:
- Taking the patient's vitals, tracking symptoms and other details of the patient's condition.
- Administering medication, change wound dressings and care for other treatment options
- Consulting with supervisors and doctors to decide on the best treatment plan for patients
- Performing physical examinations and diagnostic tests
- Responsible and accountable for the delivery and coordination of patient care and the application of the nursing process, during after-hours and weekends.
Required Skills & Qualifications:
- One year of nursing experience
Experience in home health or hospice - Provide skilled nursing services/interventions in accordance with physician orders.
- Ability to effectively communicate with patients, families, physicians and hospital staff.

- Current LVN license
- IV certified
