LVAD Patient Coordinator - Physician Assistant or Nurse Practitioner - Catholic Health

At the St. Francis Hospital Center for Advanced Cardiac Therapeutics (Roslyn, NY), an inpatient and outpatient team of board-certified heart failure cardiologists and cardiac nurses work together to deliver safe, effective and compassionate care for patients with weakened heart muscles. Our multidisciplinary approach and personalized treatment plans meet the specific health needs for patients at all stages of heart failure.

LVAD Patient Coordinator

Ventricular Assist Device/Mechanical Circulatory Support (VAD/MCS) Coordinator is an integral part of the Heart Failure/LVAD/MCS team.   The VAD/MCS coordinator helps to bridge the technical and medical aspects of the field. The VAD/MCS coordinator provides direct comprehensive patient management to patient with LVADs/MCS in collaboration with the Heart Failure attending and LVAD surgeon in both the inpatient and outpatient settings.

Requirements:

• Bachelor of Science, Masters preferred in an AMA approved PA or NP program
• NYS Physician Assistant or Nurse Practitioner License
• PA or NP Board Certification

• BLS/ACLS/AED training required and accepted from only ARC or AHA
