Service Care Manager Highlights

• The pay for this position is $22.15-$27.47 per hour.
• Remote position
• 8:00AM-5:00PM, One hour lunch break.
• This position is a temporary contract.

Responsibilities

• Assist in telehealth biannual assessments, in person as needed if in the area.
• Complete assigned tasks and document findings.
• Coordination with multi-disciplinary teams.
• Follow up calls to members.
• Any other projects that may arise.
• Must be able to meet strict turn-around times with documentation for assessments assigned.
• Address tasks in a timely manner.
• Meet with Team Lead and Manager. Trainings will need to be completed.
• Shadowing of other Service Coordinators.

Requirements

• At least 2 years’ experience as a licensed vocational nurse (LVN)
• Microsoft Word, Excel, Teams skills
• Organized, Strong time management skills
• Bilingual preferred
