About the Job
The Licensed Practical Nurse (LPN) assumes responsibilities for the direct nursing care of assigned patients under the supervision of a registered nurse (RN) in patient care departments. Provides nursing services to patients and families in accordance with the LPN scope of practice as defined by the Illinois Board of Nursing. LPN will be paired with an RN or UNL to assist with the patient load. LPN tasks may include oral meds, IV piggyback meds, trach care, dressing changes, wound care, hourly rounding, reinforcing patient education, and completion of discharges. LPNs will document assessments and care plans after the RN has completed the initial assessment and initiated the care plan.
Responsibilities:Provide direct nursing care to patients under the supervision of a registered nurse. Nursing care is guided by the provider's orders and the nursing plan of care within the LPN's scope of practice. Reports patient response of care to a registered nurse for evaluation, intervention, and modification of the plan of care. Administers medications, including IV piggybacks and fluids but excluding IV push medications. Documents assessments on assigned patients, given that an RN has charted the initial assessment. Chart on care plans, which the RN has opened. Communicate with physicians/APPs to receive orders and coordinate care for the patients. Completes dressing changes and wound care. Completes trach care. Inserts NG tubes and maintains feedings through NG tube. Cares for and changes Ostomies. Reinforces education and discharge instructions. Fulfills all requirements of departmental competencies. Takes responsibility for patient and employee safety by reporting and correcting safety hazards/incidents. Performs all responsibilities and duties required by the unit as specific to the scope of service. Works with a nurse preceptor to perform all responsibilities required by the unit and patient population. Accurately and timely, collects and documents data of nursing care rendered to assure integration of information for use by the healthcare team. Report adverse events to improve quality and safety for patients and staff. Receives constructive feedback based on performance and behavior. Regularly confers with colleagues to ask questions and expand knowledge base. Completes all mandatory training and competency modules.
Qualifications:License/Certifications:
Licensed Practical Nurse (LPN) - Illinois Department of Financial and Professional Regulation (IDFPR); Basic Life Support (BLS) within 30 days - American Heart Association (AHA)
Education:
College Diploma: Licensed Practical Nurse (Required)
