A Licensed Practical Nurse (LPN) is responsible for the supervision of staff and provides care in assigned areas. Supervises Med Techs in medication administration or administers medication directly to residents. Performs treatments, assessments (within Oregon State Board of Nursing guidelines) and documents in resident records as needed. Provides direct supervision of CMA/CNA/Caregiver personnel during the shift. Licensed Practical Nurse on duty is responsible for the wellbeing of all residents assigned to them and reports to the Director of Nursing Services.
Essential Job Functions:
- Update, reviews, and reports changes in resident condition in resident’s record, notify MD and resident’s family as appropriate.
- Directs/oversees and/or administers medications to residents.
- Observes/Assesses residents' physical, mental, and psychosocial status.
- Carries out physicians’ orders.
- Carries out recommendations by physical, occupational and/or speech therapist.
- Demonstrates knowledge and sound judgment concerning care delivery and assists with admission and discharge of residents.
- Provides resident/families with explanations of the resident's condition(s) as necessary.
- Keeps area clean and well organized.
- Reads and documents in the 24-hour report all pertinent information that needs to be communicated to the next shift.
- Fills out incident reports as required.
- Provides support to resident/families as needed.
Education:
- Completed accredited Licensed Practical Nurse program.
- Unencumbered Oregon State Board of Nursing license to practice as a Licensed Practical Nurse.
- CPR and First Aid current certification.
Years/Months of Experience:
- A minimum of 6 months working in Long-term care is preferred.
Knowledge / Mental / Skill Requirements:
- Knowledge of professional skills in organization, appearance and demeanor, written and oral communication in English.
- Communicates with discretion and confidentiality.
- Able to make quick, clear judgments based on facts and communicates issues with sensitivity to different communication styles and personalities.
- Handles high level of activity and multiple priorities in calm manner with common sense and independent action.
- Able to troubleshoot problems in medical equipment, awareness of costs of medications and supplies to help control costs.
- Supervises, trains and motivates employees to excel, promotes teamwork and quality improvement programs.
- Physical ability to operate a telephone, push/pull up to 30 pounds, carry/lift up to 25 pounds. Perform CPR.
Physical Activities of the Position:
- Able to lift assist medium (21-50 pounds) and heavy (51-100 pounds).
- Push wheelchair or geriatric chair 100 pounds +.
- Position and change residents in bed and in chairs.
