We are looking for a dedicated and compassionate Licensed Practical Nurse (LPN) to deliver outstanding care to residents in a skilled nursing and long-term care setting. Responsibilities include medication administration, health data documentation, and responding to medical and behavioral emergencies. The ideal candidate will have expertise in dementia care and a strong commitment to supporting residents' health and well-being.

Responsibilities include medication administration, health data documentation, and responding to medical and behavioral emergencies. The ideal candidate will have expertise in dementia care and a strong commitment to supporting residents' health and well-being.
Responsibilities:
- Deliver nursing care and supervision, including conducting health assessments and encouraging health maintenance
- Administer medications and perform treatments in compliance with established policies and procedures
- Gather and document residents' health and status information, ensuring effective communication
- Recognize and address acute medical conditions, implementing interventions or arranging referrals as necessary
- Act as the first responder during medical or behavioral emergencies
- Maintain precise medical records and input health data accurately
- Respond to emergencies involving aggressive or severe behavioral incidents
- Minimum of 2 years of long-term care or skilled nursing experience
- Completed dementia training
- Availability to work a rotating schedule, including every other weekend, all holidays, and flexible days off
- Experience working with individuals with intellectual and developmental disabilities a plus
- Active LPN License in MA
