We are currently in need of a
Home Health LPN Visit Nurse
in our Racine and Kenosha County territories.
Part-Time, 15-20 hours per week.
The Licensed Practical Nurse is responsible for providing direct patient care under the supervision of a Registered Nurse. Responsibilities include following the Plan of Care, providing treatments and working collaboratively with members of the team to help meet positive patient care outcomes.

The Licensed Practical Nurse is responsible for providing direct patient care under the supervision of a Registered Nurse. Responsibilities include following the Plan of Care, providing treatments and working collaboratively with members of the team to help meet positive patient care outcomes.
Duties/Responsibilities of the Licensed Practical Nurse include, but are not limited to:
- Providing direct patient care as defined in the State's Nurse Practice Act.
- Implementation of Plan of Care initiated by the Registered Nurse.
- Providing accurate and timely documentation consistent with Plan of Care.
- Collects assessment data and performs observation to repot to and collaborate with RN and physician.
- Participates in coordination of home health services, appropriately reporting the need for additional disciplines (HHA, OT, PT, MSW, ST) to the Registered Nurse and Director of Clinical Services.
- Communicates with the physician regarding the patient's needs and reports changes in the patient's condition; obtains physician's orders as required.
- Uses equipment and supplies effectively and efficiently.
- Participates in personal and professional growth and development.
- Prepares clinical notes and ensures documentation is completed on a timely basis.
- Participates in weekly staff meetings, as well as clinical in-services and skills fairs.
- Carry a current licensure in field of practice in the State of Wisconsin
- Carry a current CPR certification
- Carry a valid Driver's license and an active car insurance policy
- Excellent communication skills with patients, families, facility staff, doctors, and teammates
- Ability to work independently
- Good organization and effective time management skills
- Willingness to travel to meet patient needs
- Compliance with documentation standards and regulations
- Prior home health experience preferred, but does not affect eligibility to apply
- Flexibility in scheduling to aide in work life balance
