The Home Health Licensed Practical Nurse (LPN) is a licensed professional who administers personal care and routine skilled nursing care in the home to clients under the direction of a Registered Nurse. Collaborate with physicians, patients and their families, nurses, therapists, social workers and other members of the patient care team to implement and follow a patient specific care and treatment plan. The LPN reports to the Nurse Manager.
Responsibilities:
- Uses and/or prepares equipment and supplies effectively and efficiently adhering to aseptic technique;
- Performs other duties as assigned;
- Implements plan of care initiated by the registered nurse
- Assesses and provides patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care
- Provide accurate and timely documentation consistent with the plan of care
- Participation in Case Conferences
- Provides direct patient care as defined in the State Nurse Practice Act.
- Perform vital signs and head to toe patient assessment during visits
- Manage and perform wound care treatment (including but not limited to surgical wounds, Wound Vac)
- Provide assessment, execute and manage Central Line/Peripheral line, urinary catheters, colostomy, PleurX Drains, Lab Draws
- One year work experience as an LPN in acute, rehab or long term care.
- LPN Program Graduate
- LPN- Licensed Practical Nurse
- BLS-Basic Life Saving Certification

* Valid DL With No More Than 3 Moving Violations
