Pay: $30.00-$90.00 per visit
Schedule: Part-Time
Role Overview:
We are seeking a motivated LPN with experience in home health to join our team inWestminster and surrounding areas.
Key Responsibilities:
- Deliver high-quality, compassionate care to clients in their homes, ensuring their safety, comfort, and well-being.
- Collaborate with supervising RNs to develop, implement, and update individualized care plans based on client needs and preferences.
- Conduct regular client visits within assigned branch counties, ensuring reliability and flexibility in scheduling and travel.
- Accurately document client care activities and progress using Kantime software, ensuring timely and precise records.
- Maintain open communication with clients, families, and interdisciplinary team members to coordinate care and address any concerns or changes in condition.
- Demonstrate knowledge and compliance with IHSS regulations and procedures to ensure high standards of care.
Qualifications:
- Current LPN license in the state of Colorado.
- At least 2 years of experience in a home health care setting, with a preference for experience with both pediatric and adult clients.
- Strong ability to work collaboratively with supervising RNs and other healthcare team members.
- Reliable transportation with a willingness to travel within assigned branch counties.
- Familiarity with Kantime software is a plus; knowledge of IHSS regulations and procedures is desirable.
- Flexibility to commute from Loveland to Westminster areas and willingness to pick up skilled visits as needed.
