Licensed Practical Nurse - LPN Essential Functions:
- Conduct Home Health Aide skills trainings, competency evaluations, HHA introduction and 90 day evaluations
- Complete initial client assessment, plan of care, 90 day reassessments, and ongoing evaluations of the GAFC setting
- Partake in both care plan creations and reviews, along with discharge plans
- Assess and address motivational, psychosocial, and environmental concerns
- Maintain and update client charts, medical records and submit all relevant information to authorized organizations
- Completing notes corresponding with each on-site visit or encounter, or more often as the member's condition warrants
- Document health changes and communicate to the client's Case Manager and Program Manager in a timely manner
- Review all paperwork and plans of care to ensure program compliance, updating information as needed
- Performs any other additional duties that may be required by management at any time
Licensed Practical Nurse - LPN Requirements:
- Travel is required
- Valid drivers license and reliable transportation required
- Licensed Practical Nurse - LPN Massachusetts license required
- Minimum of 1 year experience in an acute or home health care setting or comparable work experience preferred
- Fluency in both English & second language preferred
- Case management, social services, and/or community health worker experience preferred
- Past experience working with behavioral health population is a plus
- Strong communication, writing, organizational, and time management skills
- Solution oriented with the ability to multitask
