Position Details:
We are seeking to hire a responsible, patient, and qualified Licensed Practical Nurse to assist with reviewing patient's charts. You will review patient records to ensure that information is accurate, and that records and documents accurately reflect the conditions and treatment requirements of each patient.
Responsibilities:
- Conduct comprehensive assessments of patient's medical, social, and emotional needs
- Provide case management services to ensure continuity of care and appropriate utilization of resources
- Monitor and evaluate patient progress, adjusting care plans as needed
- Collaborate with interdisciplinary teams to optimize patient outcomes
- Overseeing patient EMR to ensure that patients receive proper care
- Complete reports to reduce patient gaps and hospitalizations.
Qualifications:
- LPN license required
- Long Term Care Experience- 6 months
- EMR Experience
- MS Suite experience
- Experience with Geriatric behavioral health preferred

Salary Range: $60,000.00 - $65,000.00 per year
