We are seeking LPNs to join our fast-paced interdisciplinary team providing skilled nursing services to clients in their home. We offer an intensive mentoring program upon hire and ongoing education. The Adult Home Care team delivers care to patients in the community, providing a wide range of skilled nursing care. We are a comprehensive team comprised of RNs, Physical Therapists, LPNs, LNAs, SLPs, Social Workers and administrative support, all collaborating together to support patients to remain in living in the setting of their choice, their home.
The Community Health Nurse, LPN will be responsible for the delivery of basic nursing care to patients in a home care settings, under minimal clinical supervision, as directed by a Registered Nurse, and in accordance with an established nursing plan of care.
The general territory for this position is Burlington, Winooski and South Burlington. At times travel may be required outside of assigned territory but a general service area is assigned and part of typical travel. All mileage is reimbursed at the federal gas rate (.65c/mi)!
Qualifications:
- Accredited School of Nursing LPN Program
- Two or more years of directly related home/public health, and/or hospice experience
- Current LPN licensure or compact licensure recognized by the State of Vermont required
- Previous home care desired.
Essential Physical Requirements:
1. Must walk, sit, stand, bend, lift and move intermittently during working hours
2. Must have the ability to push, pull and lift from the floor, up to a total of 50 pounds and be able to move with the weight, as to organization’s standards
3. Climbing-be able to navigate stairs of varying heights and degrees
4. Balancing-be able to maintain balance
5. Kneeling, crouching, stooping, twisting, reaching below waist level, reaching overhead and reaching torso level
6. Manual finger dexterity-will need dexterity to perform duties and use equipment
7. Visual acuity-will need sufficient eyesight to drive a car, use office equipment and perform duties
8. Auditory acuity- will need to assist individuals in person and by telephone
