An amazing new opportunity has come up in our Long-Term Support Services Team! We are looking for a Long-Term Support Services Coordinator to join us full-time and lead our One Care LTSSC program!
Job Summary: Responsible for education and advocacy, promoting the Independent Living Philosophy, and ensuring that the Long Term Support Service (LTSS) needs of One Care members are identified and appropriately met.
Major Responsibilities:
- Participate in the initial and ongoing assessments of the health and functional status of Enrollees
- Assist in development of the LTSS component of and Individualized Care Plan
- Participate as a full member of the Interdisciplinary Care Team (ICT)
- Facilitate the provision of appropriate community LTSS and resources including Personal Care Attendant services
- Support and advocate for person-centered care planning and provision of LTSS and/or recovery needs in ways that promote independent living
- Represent the LTSS needs of the Enrollee, by identifying communication and support needs, advocating, for the Enrollee, and providing education on LTSS to the Interdisciplinary Care Team (ICT) and the Enrollee
- Monitor the appropriate provision and functional outcomes of community LTSS according to the Individualized Care Plan (ICP), upon request from the Interdisciplinary Care Team (ICT)
Qualifications:
- Bachelor’s degree in social work, human services or a related field; or at least two years’ experience working in a human service field
- Experience working with people with disabilities or elders, conducting LTSS needs assessments, and coordinating/monitoring services
- Strong interpersonal,written, verbal communication, and customer services skills
- Knowledge of home and community-based services systems and how to access/arrange services
