The Long Term Care Pharmacist would be working for a healthcare facility and has career growth potential. This is a full-time position with a schedule of Monday through Friday from 9:00 AM to 6:00 PM, and Saturdays from 8:30 AM to 5:00 PM.

Long Term Care Pharmacist Compensation
- The pay for this position is $65 per hour
- Benefits are available to full-time employees after 90 days of employment
- A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates
Long Term Care Pharmacist Highlights
- This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
- The required availability for this position is Monday to Friday from 9:00 AM to 6:00 PM and Saturdays from 8:30 AM to 5:00 PM
Long Term Care Pharmacist Responsibilities
- Provide pharmaceutical services to long-term care and institutionalized patients, focusing on maximizing quality and patient safety
- Evaluate physician medication orders for appropriateness, dosage, potential interactions, and route of administration, making recommendations to the physician and staff as needed
- Maintain patient/resident database with current and past medical history, clinical and laboratory data, and medication history
- Ensure necessary laboratory tests are ordered, evaluated, and discussed with the healthcare team
- Collaborate with healthcare team members to provide clinical and technical support
- Document all communications with physicians, nurses, healthcare team members, patients, and family members
- Implement policies and procedures for the safe procurement, storage, distribution, and disposal of drugs
- Maintain required documentation per federal and state regulations and KPS policy
- Report medication-related errors to the Pharmacy Manager
- Assist staff with procedures related to safe drug procurement, administration, and disposal
- Perform other tasks as assigned
Long Term Care Pharmacist Requirements
- Bachelor's Degree in Pharmacy require
- Pharm D preferred
- Knowledge of pharmacy laws and federal/state regulations concerning long-term care
- Ability to work cooperatively as part of a team
- Current state license in practice area required
Long Term Care Pharmacist Preferred Qualifications
- One year of experience in long-term care preferred
