Position Overview:

• Specialty: Anesthesiology
• Contract Length: Ongoing
• Start Date: January 13, 2025
• Schedule: Days, nights, and weekends; 7:00 AM - 3:00 PM with on-call coverage from 3:00 PM - 7:00 AM on weekdays. Weekend call required.
• Daily Model Options:
  • 1 MD & 2 CRNAs on Mondays, 3 CRNAs Tues-Fri
  • 1 MD Mon-Fri, 2 CRNAs Mon-Fri

Facility Details:

• Type: Critical Access Hospital
• Location: 416 E Maumee St, Angola, IN
• Patient Volume: Approximately 2800 cases/year, including general, ortho, OB, GYN, and more.

Work Requirements:

• Licensure: Indiana state license and DEA certification required (no willing to license).
• Certifications: ACLS, BLS, PALS required.
• Experience: Proficient in blocks, spinals, and epidurals; board certified preferred.

• Credentialing Timeframe: TBD, expedited for qualified candidates.
