WHAT YOU WILL DO:
The Life Coach is a non-exempt position responsible for providing support to youth and young adults ages 16-24 years old to maximize the likelihood of making a healthy transition into adulthoods.
- Establish a relationship with the consumer, family/guardian, significant other, and all other service providers connected to the consumer.
- Conducting strength-based needs assessments related to the transition domains.
- Coordinate the service planning process, with the primary goal of assisting the consumer with achieving financial independence.
- Participate in consumer staffing and follow-ups as assigned.
- Identify, deliver, create, broker, and link together the needed supports and services across the transition domains of employment, education, living situation, and community-life functioning for consumers.
- Maintain an active caseload consisting of consumers in the community and link these consumers with the necessary community support services required for maintaining the individual.
- Must ensure concise and timely completing of all paperwork or documentation, including but not limited to, administrative, clinical, and/or caseload requirements.
- Attempts to generate change and modification in client’s behavior and attitude to facilitate personal growth, independence, and recovery.

WHAT YOU WILL NEED:
- Bachelor's degree in human services, psychology, social work, sociology, criminology or any related field
- One (1) year working with youth or relevant case management experience.
- Experience working with youth or young adults coping with trauma and behavioral health issues and homelessness strongly preferred.
- Working knowledge of community organizations and resources.
- Attend and complete Transition to Independence Process (TIP) Model Core Competency Training.
- Must have a valid drivers’ license with auto insurance coverage.
