Position Overview:
We are seeking a compassionate and dedicated Licensed Vocational Nurse (LVN) to join our team at our esteemed Skilled Nursing Facility. As an LVN, you will play a vital role in providing high-quality care to our residents, ensuring their physical and emotional well-being. Your expertise, professionalism, and strong interpersonal skills will contribute to creating a warm and supportive environment for our residents.
Key Responsibilities:
Resident Care: Administer comprehensive nursing care to residents under the supervision of a Registered Nurse (RN) or licensed physician. Assist residents with daily activities, such as bathing, dressing, grooming, and feeding, while promoting independence and maintaining their dignity.
Medication Management: Administer medications as prescribed by physicians, adhering to established protocols and accurately documenting each administration. Monitor medication effects and promptly report any adverse reactions or changes in residents' conditions to the RN or physician.
Vital Signs and Assessment: Regularly monitor and record residents' vital signs, weight, and other health indicators. Perform basic assessments, document findings, and collaborate with the healthcare team to develop care plans.
Wound Care and Dressing Changes: Provide basic wound care and change dressings as required. Maintain a clean and infection-free environment to facilitate healing.
Collaborative Communication: Maintain open and effective communication with residents, their families, and the interdisciplinary care team. Report any pertinent information or concerns to the appropriate personnel promptly.
Record Keeping: Ensure accurate and timely documentation of resident care, assessments, medication administration, and other required records using electronic health record systems or other designated tools.
Infection Control: Adhere to infection control protocols and maintain a clean and safe environment for residents and staff. Comply with facility policies and procedures to prevent the spread of infections.
Emergency Response: Respond promptly and efficiently to emergencies, such as falls or sudden changes in residents' conditions. Initiate appropriate interventions and inform the appropriate authorities.
Supportive Services: Participate in activities that enhance the overall well-being of residents, such as recreational programs and social events. Encourage resident engagement and social interaction.
Compliance: Ensure compliance with state and federal regulations, licensing requirements, and facility policies and procedures.
Qualifications and Requirements:
Valid LVN license in the state of California.
Graduation from an accredited vocational nursing program.
Current CPR certification.
Prior experience in a skilled nursing facility or a similar healthcare setting is preferred.
Strong clinical skills and a thorough understanding of nursing practices and procedures.
Ability to work collaboratively as part of an interdisciplinary team.
Empathetic, patient-centered approach to care.
Excellent communication and interpersonal skills.
Detail-oriented and proficient in maintaining accurate records.
Knowledge of infection control protocols and safety guidelines.
