Observe each resident on a continuing basis alert to changes in mental and physical well-being; notifies the Supervisor of pertinent observations and discusses the need to notify physician; and receive Resident Status Report from off-going shift and give on-coming shift a detailed report of each resident s status and events of the shift.
Accurately receive, transcribe, and carry out physician s orders.
Administer medications and treatments as ordered by the physician and make proper documentation via a computerized system of MARs/TARs and electronic documentation. Order and maintain all pharmaceuticals for the units.
Supervise LNAs in accordance with licensing requirements under the New Hampshire Board of Nursing. Assign and measure nursing staff resident care consistent with abilities, job description and scope of practice and supervise the care given; and assist direct-care staff with nursing care as necessary and as time allows.
Accompany physician on resident rounds, on their assigned unit(s), to address residents medical needs and to facilitate communication during 60-day review.
Administer the provision of oxygen in accordance with facility guidelines and procedures. Administer gastrostomy/jejunostomy feedings in accordance with policy and procedure. Perform wound care and have knowledge of wound vacs.
Perform regular duties on the unit such as checking appointments for the day; checking Physical, Occupational and Speech Therapy appointments; checking activity board; reminding staff of ambulation list, weights, blood pressures, and to document nutritional charts. Monitor LNA documentation and update LNA clipboard; oversight of infection control practices on the unit; ensure that unit is clean and hazard-free. Following the chain of command, report any problems to the Nurse Manager/Nursing Supervisor. Encourage LNA observations and include them when documenting and reporting.
Admit, discharge and transfer residents, completing documentation and notifications as necessary.
Complete 24-hour report for his/her assigned unit(s); check new orders; complete list of residents on antibiotics, precautions, etc.; maintain consistent communication with the Nurse Manager/ Nursing Supervisor regarding resident issues or concerns with staff; differentiating between acute and non-emergency problems; and attend all mandatory staff/educational meetings and non-mandatory meetings as assigned.
Communicate with residents families to resolve resident concerns and problems to notify family members of new orders, and to obtain consent when necessary.
Receive incoming calls and determine appropriate response required.
Assume responsibility for the management of assigned unit(s) during his/her shift under the direction of the Nursing Supervisor (NS) or Director of Nursing Services (DON).
Follow State and Federal Regulations regarding the storage, usage and documentation of controlled medications including counting at the change of shift; notifies appropriate Supervisor if discrepancies are present.
Assist with the orientation of new nursing employees on their assigned unit(s). Communicate with DON, Nurse Manager or SDC regarding continuing education needs of the nursing staff and assist in the delivery of staff education and training as needed. Encourage and assign staff to attend in service education meetings. Evaluate assigned nursing staff members according to facility policy and procedure. Initiate or effectively recommend disciplinary action as necessary.

The physical, emotional, spiritual and psychological needs of each resident are addressed by qualified medical nursing staff and supportive allied health care professionals while maintaining each resident's rights of confidentiality, safety, and quality of life. Our home is Medicaid licensed for 156 beds as an Intermediate Care Facility (ICF) and is certified by the New Hampshire Bureau of Health Facilities Administration. The nursing home provides 24-hour Nursing care and other services to residents whose health care needs cannot be successfully managed at home or in a community setting.
Sullivan County Health Care is also a Medicare approved Skilled Nursing Facility (SNF). If a potential residents doctor determines that Skilled-nursing placement is appropriate, following a qualifying stay in the hospital, we can accommodate their needs.
Once the facility application is completed in full and returned, medical information will be obtained to assist in determining nursing home placement. If necessary, a home visit will be scheduled with our Director of Community Development. If an appropriate bed is not available, a waiting list is maintained, with Sullivan County residents having priority.
