Job Summary: Located in beautiful Delta, CO, the Licensed Therapist position is responsible for providing effective behavioral health therapy to patients with medical disorders through brief encounters with patients, co-visits with medical providers and longer counseling sessions. This position will deliver and document individual behavioral health encounters that support health conditions, crisis management, behavioral health case management, care coordination and consultation. This is a salaried exempt position, and comes with a $10,000 sign on bonus, to be paid out over the first year of employment.

Responsibilities:
Accurately identify behavioral health needs.
Build rapport with patients that induces a relationship of engagement and trust.
Encourage patients to become an active part in their health care plan.
Implement care plans that address behavioral health needs alongside a team of providers.
Process referrals and deliver patient centered, culturally sensitive evaluations that meet the needs of the primary care setting.
Monitor patient progress by recording documentation in health records.
Recognition of changes over time that would alter a care plan or treatment.
Communicate effectively with patients, providers and staff.
Improve care team functions and relationships by observing opportunities.
Facilitate referrals and connections to community resources and other specialists.
Knowledge of health-related information related to all ages.
Knowledge and experience working as a behavioral health specialist.
Knowledge of parenting and coaching tactics.
Knowledge of compliance with applicable federal, state, and local laws, professional practice acts and ethical guidelines, policies and procedure.
Skill in thinking in terms of population management.
Skill in assessing levels of risk and making clinical judgement for resolutions of patient emergencies.
Skill in various computer programs and medical records systems.
Skill in handling sensitive and confidential matters with the utmost discretion.
Ability to understand and use collaborative documentation when clinically justified.
Ability to work independently as well as cooperatively as part of a team.
Ability to produce written documentation of clinical work in a timely manner which is understood by other medical providers.
Ability to utilize collaborative documentation when clinically justified and as requested by the patient.
Education: Master’s Degree in Counseling or Social Work
Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC)
Equipment Operated: Standard office equipment including computers, fax machines, copiers, printers, telephones, and calculator.
Work Environment: Position is in a well-lit office environment.
Physical Requirements: Must possess the physical and mental abilities to perform the tasks normally associated with Integrated Behavioral Health. Daily activity is 90 percent sitting and 10 percent walking or standing. Dealing with a wide variety of people on various issues may cause stress.
