Position: Local Clinical Specialist (LPN or RN)Location: District 6 (Cambridge City , IN or Tipton, IN)Duration: 12 Months (Extendable)Webcam or In Person InterviewPurpose of Position/Summary:To Provide nursing care services to Indiana residents that follow infectioncontrol practices as guided by the CDC, CMS and state regulations. This positionwill provide essential assistance during periods of increased patient volume oremergencies.Essential Duties/Responsibilities:• Provide onsite point of care services such as vaccinations and testing. Followall agency and division policies and procedures relating to safety, infectioncontrol, documentation, and eligibility. Document and report concerns asdirected to agency leadership.• Maintain proficiency in all related nursing skills. Complete all requiredtrainings and in-services and maintain all required licensure andcertifications.• Maintain confidentiality of all patients.• Complete required documentation related to all aspects of care provided.• Document findings and submit reports within required timeframes.• Attend required staff and Division meetings.• Other duties as assignedJob Requirements:• Must at least hold an LPN license; RN license preferred. Must be knowledgeablein infection control practices, vaccinations, point of care testing, andphlebotomy. Will travel to various locations in Indiana and assist in theoperation of mobile clinics;• Specialized knowledge of principles of nursing practice and standards of care;• Ability to work without onsite clinical supervision;• Knowledge of nursing procedures and the ability to work in a variety ofsettings;• Ability to identify potential adverse health and safety conditions;• Ability to work as part of a team;• Ability to utilize computer equipment and software to enter, retrieve andmanipulate data;• Must hold and maintain a Valid Indiana’s Driver’s License;• Ability and willingness to work before and after standard work hours,including on weekends;• Professional demeanor and ability to maintain composure under stressfulsituations;• Commitment to the public health mission of protecting the health, safety,rights, privacy, and well-being of residents in long term care facilities;• Effectively communicate, both orally and in writingSupervisory Responsibilities/Direct Reports:Not applicableDifficulty of Work:Must be able to travel throughout an assigned region of Indiana. The idealcandidate would be willing to respond to surges in patient demand by beingavailable for additional shifts or on short notice.Might have some overnight stays depending on location of outbreaks, and agencyneeds.Responsibility:The public health nurse plays a crucial role in community well-being.Responsibilities may include conducting health assessments, providingvaccinations, testing, offering health education and providing treatment andcare within diverse populations. They are required to provide care, anddocumentation consistent with all state and federal rules/regulations.Personal Work Relationships:This individual may be expected to work alone or within team and will partnerwith agency personnel and related stakeholders working towards completion ofassigned functions. Exceptional customer service is provided to agency partners,facility partners, and/or the general public.Physical Effort:• This role requires the ability to operate a motor vehicle and drive short andlong distances in all weather and light conditions.• Depending on the duties performed or environment setting, frequent walking,standing, climbing stairs, bending, stooping, pushing, pulling and/or similarmovements may be involved.• This role requires the ability to lift/transport items up to 20 lbs with orwithout the assistance of equipment.• This role requires keen observational and hearing skills to stay alert forpossible dangerous situationsWorking Conditions:Conditions may vary depending on clinic location. Clinics may be held outdoorsin a variety of weather conditions. During outbreak response, all PersonalProtective Equipment (PPE) must be worn according to agency policies andinfection control practices must be followed at all times.• Work will be accomplished as part of a larger mobile team.• Individual will be assigned to a specific district mobile unit.• Work will begin and end at a predetermined location.• Personnel will ride to and from sites in the mobile unit unless otherwiseauthorized by direct supervisor.• Agency will provide all supplies and equipment necessary to perform assignedtasks.
