Licensed Practical Nurse Rehabilitation at Clinical Management Consultants

The Licensed Practical Nurse Rehabilitation will report directly to the Nurse Manager, with oversight from the Director of Nursing. This role collaborates closely with Registered Nurses (RNs), Certified Nursing Assistants (CNAs), and other healthcare professionals to ensure comprehensive care for our patients.

Our rehab facility provides a wide range of services, including post-surgical care, physical therapy, and long-term care. The unit is designed to help patients regain independence and return to their daily activities. Job Responsibilities

• Provide direct nursing care to patients, including administering medications, monitoring vital signs, and assisting with daily activities.
• Collaborate with RNs and other healthcare team members to develop and implement individualized care plans.
• Document patient progress and communicate any changes in condition to the healthcare team.
• Assist with rehabilitation activities, encouraging patients to participate in their recovery process.
• Educate patients and families about care plans, medication management, and post-discharge care.
• Maintain a clean and safe environment, adhering to all facility policies and procedures.
