Community Care, Inc. is currently seeking a reliable LPN to provide in home care to a 7 year old boy in the Vanderbilt area. Nurse would be responsible for all ADLS and medication administrations.
Shifts Available:
- Monday - Friday 8a-4p
- No weekends!
POSITION SUMMARY: The LPN works under the guidance of the RN as a member of a team to provide direct nursing care to patients within the policies of the agency. The activities of the LPN are based on the employee's educational preparation, experience and competency evaluation. The LPN does not carry out admission, recertification, resumption of care or transfer/discharge assessments.
RESPONSIBLE TO: Director of Professional Services or designee.
I CLINICAL
- Implements and evaluates all skilled and technical patient care activities designated by the RN serving as case manager or RN supervisor.
- Implements nursing care plan under the direction of the RN, making care plan suggestions as dictated by client conditions and maintaining patient safety.
- Administers prescribed medications (must have training in pharmacology and demonstrated competence in medication administration). See Medication Administration Policy.
- Performs specific treatment as ordered by the physician under RN supervision.
- Makes observations related to the patient's condition, home situation and treatment response.
- Accepts verbal physician orders after policy orientation and within regulatory and agency policies. See Nursing Service Policy, Physician Plan of Care and Certification Policy.
II DOCUMENTATION
- Completes the electronic shift note and/or nursing visit note and related recording on the day of the visit, documenting the treatment provided and the patient's response.
- Submits the clinical notes timely utilizing an electronic medical record.
- Completes the Time Slip recording as indicated by RN.
III COMMUNICATION
- Reports observations of patient's clinical changes, treatment responses and home situation to the supervising RN and/Case Manager.
- Maintains confidentiality of client information adhering to HIPAA regulation.
- Coordinates physician-ordered patient care activities with the RN Field Supervisor.
- Inventories and orders supplies as needed.
- Provides CCI with monthly availability in a timely manner.
IV PROFESSIONAL STANDARDS
- Maintains a professional standard of conduct and projects a positive image of the agency at all times.
- Complies with infection control and safety policies and procedures.
- Adheres to all policies and practices of the agency to maintain established Standards for Care.
- Adheres to all policies set forth in the Nurse Practice Act to maintain safe clinical practices.
- Demonstrates effective time management.
V PROFESSIONAL DEVELOPMENT
- Attends annual mandatory in·service education sessions.
- Participates in additional staff meetings, care conferences, required educational sessions and the Performance Improvement Program, as appropriate.
- Continually strives to improve care by broadening and deepening knowledge through formal education, attendance at workshops, conferences, active participation in professional and related organizations and individual studies and readings.
VI PERFORMS OTHER DUTIES AS ASSIGNED
VII QUALIFICATIONS
- Graduate of state-approved school with course in pharmacology.
- Current license in the State of Pennsylvania.
- Minimum of six (6) months experience in relevant patient care.
- Current CPR Card.
- When required to travel from one assignment to another on the same day, a reliable independent means of transportation is required. Drivers must have a valid Driver's License and car insurance limits adhering to the limits established in the personnel policies.
- Obtains and retains a clear criminal history, Childline Clearance, FBI Clearance, DIG, Medicheck, EPLS and any additional required regulatory clearances.
