S:US is seeking a LPN for their adults family shelter in Manhattan.
This position is located at East 31 Street New York, NY 10016
The LPN meets with the Program Director, Director of Social Services, Registered Nurse, and Social Service staff to coordinate shelter residents to receive ongoing treatment in the community.
The LPN will;
- Take vital signs, such as blood pressure, temperature and weight
- Assist with diabetes medication and treatment
- Take medical histories and enter information in computer systems
- Coordinate with Case Managers, Senior Case Managers and Director of Social Services to ensure proper care, nutrition and treatment plans are followed
- Follow up with community health care providers to ensure medical treatment plans are documented, monitored and followed
- Have flexibility to work a schedule that includes evenings and weekends
Qualifications
- Completion of accredited program and licensed in New York as an LPN.
PREFERRED QUALIFICATIONS & SKILLS
- Ability to work independently and as part of a team, to assist adult males of diverse ethnicities, backgrounds, ages, and sexual preferences. Bilingual preferred.
